What is an Acceptance Agent?
A Passport Acceptance Agent verifies your identity, witnesses the signing of the DS-11 application, and seals the application packet in an official government envelope. There are over 7,500 facilities nationwide — USPS post offices, clerks of court, public libraries, and local government offices.
Who needs to visit one?
- First-time passport applicants
- Children under 16
- Anyone replacing a lost, stolen, or damaged passport
- Anyone whose last passport was issued more than 15 years ago
Not required for standard adult renewals (DS-82).
What to bring
- Completed but unsigned DS-11 form (2 printed copies with barcode)
- Proof of US citizenship (original birth certificate)
- Valid photo ID
- Passport photo (2″ × 2″)
- Signed Letter of Authorization (LOA)
- Payment for the $35 acceptance fee (check or money order)
